When you’re living with erectile dysfunction (ED), you’ll consider doing just about anything to get your sex life back on track. Eating roots like ginseng — fine. Drinking special juices like pomegranate — tasty. Even becoming a human pin-cushion for a treatment like acupuncture — bring it on.

But do any of these things really work? Eric Laborde, MD, a spokesman for the American Urological Association, is skeptical.

“There is a huge placebo effect with erectile dysfunction,” he says. “I could give you anything and tell you it’ll work and it will in the short term.” For that reason, he says, what little studies that have been done on herbal remedies are less than convincing.

On the other hand, Geo Espinosa, director of the Integrative Urological Center at NYU Langone Medical Center, says there are some natural alternatives to popular ED medicines. Take them with your doctor’s OK, and they can help you get and keep an erection.

“It’s not like you go into the forest and just eat a leaf,” he says. “You don’t take them right before sex. But there are real natural approaches that work by building up sexual health over time.”

These natural alternatives to ED drugs are widely used. Each comes with its own set of pros and cons.

Acupuncture: This traditional Chinese needle therapy is said to boost blood flow. It’s most often used to treat chronic headaches and back pain. But some men have found it works for erectile dysfunction.

While some studies back up acupuncture’s claim to work for ED, the evidence is rather scant.

Arginine: It’s found naturally in food, and it relaxes blood vessels and is used to treat heart problems. Doctors think it boosts circulation to the penis, but Espinosa says he doesn’t recommend it much because the body metabolizes it so quickly. “You need a lot and you need to take it frequently for it to work,” he says.

Instead, Espinosa suggests taking L-citrulline, which helps your body make more arginine. But, he warns, high doses can lower your blood pressure.

DHEA: The hormone that your body converts into testosterone might sound like the most natural salve for a lagging love life. Sometimes it can help ED, but that’s not always the case.

“If your testosterone is low, then that might be playing a role” in your erectile dysfunction, Laborde says. “But if it’s normal, making more testosterone is not going to help you.”

Ginkgo Biloba: One of the best-selling herbal remedies in the U.S. and Europe, extracts from this species of tree can boost blood flow. It’s widely used to treat blood disorders and memory issues, but some people use it to treat ED.

“The downside is: It just doesn’t work,” Laborde says. Meanwhile the possible side effects can include increased bleeding, convulsions, diarrhea, and upset stomach.

Espinosa says he recommends ginkgo more for the brain than for erectile dysfunction.

Ginseng: Korean red ginseng is a widely used herbal ED remedy. That’s partly because it’s been shown in studies to do some good — even though researchers still can’t quite explain how it works.

“If someone really wants an herbal supplement, I’ll recommend ginseng,” Laborde says. “At least it’s not going to hurt you.”

Horny Goat Weed: Extracts from the leaves of this herb have long been used as an ED remedy in traditional Chinese medicine. At least one study on rats showed promise, but it has yet to be tested on people.

Espinosa says the herb generally boosts energy, and he recommends it more to balance out your whole body as opposed to targeting ED. It has been linked to low blood pressure.

Pomegranate: Juice and extract from this fruit is known for being packed with nutrients. But Espinosa says it can also help arteries expand and remain supple. While no real proof ties it to erectile dysfunction, he suggests it to his patients for its overall health benefits.

Rhodiola Rosea: Also known as “golden root,” this plant is a relatively new herbal remedy in the U.S. It’s been used to boost endurance and sexual function in Russian folk medicine for decades.

Espinosa says it can ease symptoms of exhaustion and ED. And it’s less likely to leave you feeling wired than some other products. The side effects are minimal, he says, both in his experience and in the studies he’s seen.

Yohimbe: For years the bark from this African tree was a go-to supplement for ED. Some clinical trials have shown improvement for people who have sexual problems due to depression meds.

But the list of side effects includes increased blood pressure, fast or irregular heartbeat, and anxiety. Make sure you talk to your doctor before you try it.

What Is Viagra?

Viagra is a pharmaceutical drug available by prescription in most areas. A prescription is given to a patient when they have been diagnosed with erectile dysfunction. Viagra has a physiological effect in the body, essentially allowing blood to flow into the penis more effectively.

This medication is classified as a PDE5 inhibitor. The drug class inhibits a specific enzyme that is called PDE5. The drug is only advised for men, and a doctor will generally first consider the patient’s overall health before prescribing the medication.

The purpose of Viagra is to make the process of getting an erection faster and easier for men with erectile dysfunction. The drug will not cause a spontaneous erection, however. Men need to be sexually stimulated for an erection to occur.

In addition to helping a man get a harder erection, Viagra is also a practical option for increasing the duration of an erection. With erectile dysfunction, many men are unable to maintain their erection long enough for sexual intercourse.

The drug takes about 30 minutes to start working, but in some men, about an hour is needed before the effects can be felt. It is generally taken as needed. Most men take Viagra an hour before they will have sex.

Arginine (or l-arginine) is an amino acid found in animal products, beans, and whole grains. Multiple studies have consistently found it can significantly improve ED, making it one of the most commonly used natural viagra alternatives.

Arginine boosts levels of nitric oxide (NO), a compound that triggers muscle relaxation, allowing for increased blood flow to the penis. That increased blood flow allows for an erection, says Dr. Deborah Matthew, a practictioner at Signature Wellness,

How to use it: A medical review found that 1,500 mg to 5,000 mg doses of arginine were effective for treating ED. Arginine supplements are generally safe, Matthew says, but may pose a risk for people with heart disease, low blood pressure, or herpes.

2. Horny goat weed

Horny goat weed refers to a family of herbs called epimedium. It’s been used for centuries in traditional Chinese medicine to treat sexual health issues in males and females.

Modern research suggests that icariin, a component of horny goat weed, inhibits the production of the phosphodiesterase type-5 (PDE5) enzyme, Stahl says. That allows for more NO and stronger erections.

This mimics some of the effects of front-line prescription treatments for ED — including Viagra, Cialis, Levitra, and Stendra — which are also PDE5 inhibitors.

How to use it: Horny goat weed can be taken in daily 2,000-mg doses. Or, you can take 3,000 – 4,000 mg 90 minutes before having sex.

If you have cardiovascular disease or estrogen-sensitive health conditions like breast cancer, don’t take horny goat weed, Stahl says.

3. Beets

“Beets are like nature’s Viagra,” Matthew says. They work in three ways:

  1. Increase NO levels: Studies on consuming beetroot juice or taking a beetroot juice supplement found that both can increase NO levels for better blood flow.
  2. Reduce blood pressure: Beetroot juice can also reduce blood pressure, according to a 2022 medical review. Men with high blood pressure are twice as likely to have ED.
  3. Boost athletic performance: The increase in NO levels from beets may also help you last longer during sex by upping your athletic stamina.

So, while there hasn’t been a specific study on beetroot juice and ED, it’s likely to have benefits, Matthew says.

How to use it: There’s no proven dosing information on beetroot, so talk to your doctor. Consider eating more beets or incorporating a beetroot juice supplement powder into your diet.

4. Red ginseng

Red ginseng, also known as Asian ginseng, is a root that is used to treat erectile dysfunction in traditional Chinese medicine.

Modern research and small studies show that men who take red ginseng may have a stronger sex drive and fewer erectile problems compared with men who took a placebo.

For example, a 2012 study of 119 men who took 1400 mg of red ginseng per day for eight weeks had slightly less erectile dysfunction than men who took a placebo.

How to use it: Stahl recommends taking a 600-1,000 mg dose of red ginseng each day. Don’t try this if you are taking:

  • Blood thinners
  • Blood pressure medicine
  • Insulin for diabetes
  • Immune system suppressants
  • Monoamine oxidase inhibitor (MAOI)
  • Stimulants

5. Tribulus terrestris

Tribulus terrestris is an herb that has shown some promise for treating ED, possibly because it affects NO levels.

A 2017 study on the topic of tribulus terrestris and ED examined 180 men and found that those who took six 250-mg doses of tribulus terrestris daily for 12 weeks had increased sexual satisfaction and lower rates of ED than men who took a placebo. The study found no adverse side effects.

How to use it: The men in the study took six 250-mg doses a day, but there is no FDA-approved dose for tribulus terrestris. Talk to your doctor about what’s right for you.

Don’t take tribulus terrestris if you’re on medication for diabetes or high blood pressure, or if you’re taking diuretics or clopidogrel, a drug that treats cardiovascular disease.

6. Therapy

Psychological and emotional concerns like stress, relationship issues, or anxiety can cause ED or make it worse. In those cases, therapy is a helpful natural treatment, Stahl says.

A small 2020 study found that cognitive behavior sex therapy helped treat ED. Other types of therapy can be beneficial too, Stahl says.

“Couples therapy, sex therapy, or cognitive behavioral therapy can be effective,” he says.

How to use it: Reach out to a qualified mental health provider who has experience working with people with ED. At the same time, try using the erection confidence app to reduce performance anxiety and help you have better sex.

Vitamin D

Multiple studies have shown that many people who experience erectile dysfunction are also vitamin D deficient. A 2018 study suggested that achieving healthy levels of vitamin D could improve testosterone levels, thereby increasing erectile function.

A 2020 meta-analysis , which looks at many studies, concluded that severe forms of ED are associated with a vitamin D deficiency. Another 2020 review of studies agrees, stating, “Evidence showed that vitamin-D is needed for an adequate erectile function. Briefly, vitamin-D is crucial for a better healthy body and sexual activity.”

Additional research is needed to understand how effective vitamin D is when used to treat ED. But, even if a vitamin deficiency does not directly cause erectile dysfunction, there are plenty of benefits to a vitamin D supplement. If you have low vitamin D levels, upping your vitamin D intake can help foster a healthy immune system and support bone health.

Vitamin D dosage for ED

Check with your healthcare provider regarding your vitamin D levels. Levels of vitamin D can be easily checked with a blood test. Based on the results, your healthcare provider will recommend an appropriate dosage for you. Some people need a higher dosage, for example a once-weekly dose of 50,000 IU (international units), while others will take a smaller daily dose.

Vitamin D-rich foods

Aside from getting enough sunlight (fun fact: vitamin D is also known as the “sunshine vitamin”) other good sources of vitamin D are:

  • Fatty fish and seafood like salmon, mackerel, sardines, tuna, shrimp, and oysters
  • Cheese
  • Egg yolks
  • Mushrooms
  • Vitamin-D fortified foods including cow’s milk, nut milks, orange juice, and yogurt

Although vitamin D toxicity is rare, if you have too much, you may experience extreme side effects like kidney damage and heart rhythm issues.

Vitamin B3 (niacin)

Vitamin B3, otherwise known as niacin, is one of the few vitamins with promising research some efficacy in boosting erectile function. That said, the sample size of the existing studies has been very small. Research with larger sample sizes is required.

Niacin is known to help decrease high cholesterol levels, and may also help reduce blood pressure, both of which can contribute to impotence. One study concluded that in patients with ED and high cholesterol, niacin helped improve erectile function. If these health problems are the underlying cause of your ED, niacin may help increase circulation to your penis so you can achieve an erection.

Vitamin B3 dosage for ED

In the above-mentioned study, patients took up to 1,500 mg of niacin per day. However, this is a very high dose, and a lower dose, such as 500 mg, may be sufficient. Ask your healthcare provider for a dosage recommendation. Also, niacin is known to cause flushing. You may want to look for a sustained-release or flush-free form of niacin ( however , the flush-free form may not be as well absorbed).

The most common side effect of niacin is flushing of the skin. Starting at a lower dose and slowly increasing the dose may help; some people find that taking aspirin (if tolerated) helps with the flushing as well. Other adverse side effects of consuming too much niacin include blurred vision, nausea, liver issues, and gout.

Vitamin C (ascorbic acid)

There is no evidence that vitamin C is effective at directly improving erectile dysfunction. In order to get an erection, your body needs to release nitric oxide . Vitamin C does not release nitric oxide directly, but it can support the biochemical pathways necessary for its release (along with folic acid, calcium, and vitamin E). A prescription phosphodiesterase type 5 inhibitor, like Viagra, may enhance the benefits of vitamin C.

There are many other reported benefits of getting enough vitamin C through vitamin C supplements and/or diet. It’s necessary for the growth, repair, and development of all body tissues. In other words, the risk of trying it is low. At the very least, it will help boost your immune system .

Vitamin C dosage for ED

There is no standard dosage of vitamin C for ED. A general rule of thumb is 90 mg per day for adult men, which you can easily get in your diet (or with a vitamin if you prefer). You can ask your healthcare provider to recommend a dose appropriate for you.

While human studies haven’t shown that it can raise testosterone levels, it appears to increase sex drive in people of both sexes. In fact, in a 90-day study in women reporting low sexual pleasure, taking 750 mg of tribulus daily for 3 months increased sexual satisfaction in 88% of participants ( 2 ).

However, the study did not include a placebo group, so no conclusions can be drawn.

However, studies in people with ED show mixed results.
