What is Viagra®?
Sildenafil, Revatio®, and Viagra® are the same thing, and all work by the same mechanism. As a potent PDE5 inhibitor, sildenafil increases blood flow, especially in the penis. You can achieve the same chemical effect with:
voriconazole will increase the level or effect of sildenafil by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Use Caution/Monitor.
efavirenz will decrease the level or effect of sildenafil by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Use Caution/Monitor. Potent CYP3A4 inducers are expected to cause substantial decreases in sildenafil plasma levels
This is where some people get confused. Is Revatio® the same as Viagra®? Chemically, yes it is, and this is commonplace in the world of pharmaceuticals.
chloramphenicol will increase the level or effect of sildenafil by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug.

OVERDOSE: If someone has overdosed and has serious symptoms such as passing out or trouble breathing, call 911. Otherwise, call a poison control center right away. US residents can call their local poison control center at 1-800-222-1222. Canada residents can call a provincial poison control center. Symptoms of overdose may include severe dizziness, fainting, painful/prolonged erection.
sildenafil increases effects of alfuzosin by pharmacodynamic synergism. Avoid or Use Alternate Drug. Risk of hypotension; separate sildenafil >25mg from alpha blocker by 4hr.
esmolol increases effects of sildenafil by additive vasodilation. Use Caution/Monitor. Sildenafil has systemic vasodilatory properties and may further lower blood pressure in patients taking antihypertensive medications. Monitor blood pressure response to sildenafil in patients receiving concurrent blood pressure lowering therapy.
In a systematic review, treatment with sildenafil for 12 weeks or more improved clinical symptoms of pulmonary hypertension. Sildenafil was also found to help delay the progression of the disease in adults.
According to a similar review of men with erectile dysfunction, sildenafil was found to be effective and well-tolerated. Compared to placebo, or no treatment at all, sildenafil helped improve erectile dysfunction with generally mild side effects. Sildenafil may especially be useful for men with Peyronie’s disease, a penile tissue disorder where scar tissue can cause the penis to bend in an abnormal way.

Although rare, some users of Revatio or Viagra have experienced a condition called anterior ischemic optic neuropathy (AION). This condition is characterized by a decrease or sudden loss of vision. Another related adverse side effect is the decrease or sudden loss of hearing. These adverse side effects have also been seen with other PDE-5 inhibitors.
Serious – Use Alternative (1) lonafarnib will increase the level or effect of sildenafil by affecting hepatic/intestinal enzyme CYP3A4 metabolism. Avoid or Use Alternate Drug. Avoid coadministration with sensitive CYP3A substrates. If coadministration unavoidable, monitor for adverse reactions and reduce CYP3A substrate dose in accordance with product labeling.
Revatio and Viagra contain the same active ingredient—sildenafil. However, they both contain different strengths of sildenafil. Revatio is taken at a lower dose three times per day while Viagra is taken at higher doses not more than once per day.
Off-label uses of sildenafil include treatment of high altitude pulmonary edema. High altitude pulmonary edema occurs when fluid builds up in the lung at certain altitudes. In those who experience Raynaud’s phenomenon, sildenafil may be considered for treatment. Raynaud’s phenomenon is characterized by the discoloration and loss of sensation of the fingers and toes in certain temperatures. Sildenafil is only recommended as an option when other treatments are not available or have already been tried.
Viagra® worked well for these men, and it quickly turned into a multi-billion dollar product for Pfizer. But the company never lost sight of their original goal: a drug for hypertension, and in 2005 sildenafil was approved by the FDA for the treatment of pulmonary arterial hypertension.
The list of the common side effects that are associated with sildenafil is pretty small as it’s considered to be a fairly safe medication. The most common side effects are headaches, indigestion and flushing (warmth and redness of the face or body). Most of these conditions are only felt by around 20% or fewer of patients so even though they are the most common, they are still relatively rare.

Sildenafil has also been shown to help treat women who experience female sexual arousal disorder (FSAD). This disorder is common in up to 26% of American women.
The most notable decreases in blood pressure for those with PAH can be seen within 1 to 2 hours after administration with Revatio. Sildenafil and its active metabolite have half-lives of about 4 hours. For this reason, Revatio should be taken every 4 to 6 hours.
Viagra®, or its nickname “little blue pill” may suggest, is much more well known than Revatio since more men are affected by erectile dysfunction than they are by pulmonary arterial hypertension. It’s estimated that roughly 30 million men are affected by erectile dysfunction in the United States alone so Viagra® will have many more users than Revatio. Sildenafil can also be used to treat a few other conditions as well:
Riociguat is a drug that can treat pulmonary arterial hypertension. Because of its possible hypotensive effects, it is not recommended to be used when taking sildenafil.
Viagra is taken at a higher dose not more than once per day. It is usually taken between 30 minutes and 4 hours before sexual activity.
Roman-affiliated doctors may prescribe VIAGRA® or generic VIAGRA® for the treatment of premature ejaculation (PE), if they believe in their medical judgment that it is an appropriate course of treatment. While this is not an FDA-approved use of the drug, the American Urological Association has included the use of sildenafil citrate in the treatment of PE in its Guideline on the Pharmacologic Management of Premature Ejaculation.
Less commonly, these medications may cause serious side effects. Seek emergency medical care if you have an erection that lasts longer than four hours (priapism), trouble hearing, or sudden vision loss.
Note that sildenafil may not be safe for everyone. Your healthcare provider may not prescribe this medication if you have certain medical conditions, such as cardiovascular (heart) disease, low blood pressure, vision problems, sickle cell anemia, or a recent heart attack.
